Objectives: To identify the association between xerostomia and hyposalivation and its risk factors in people aged 60 years and older, and to investigate the association with medications, habits and other oral complications. Methods: 211 participants were included. Xerostomia data was collected using a validated Spanish-Xerostomia Inventory (XI-sp). Unstimulated whole-salivary flow rates were measured to detect hyposalivation. Results were analyzed using Chi-square, Fisher tests and multivariate logistic regression analysis. p &lt;.05. Results: No significant association was detected between Xerostomia and Hyposalivation (p=.0666). Xerostomia 84.3%(p=.036) and hyposalivation 81.4%(p=.004) occur more frequently in women. A significant association was found between hyposalivation with the female gender (OR = 2.46, 95% confidence interval (CI): 1.19-5.11, p=0.015); denture stomatitis (OR=3.71, 95% CI:1.03-13.3, p=0.045) and atrophic glossitis (OR=3.72, 95% CI:1.78-8.1, p=0.001). Only female gender (OR=2.54; 95% CI:1.19-5.43, p=0.016) was significantly associated with xerostomia. Conclusions: No statistically significant association was found between hyposalivation and xerostomia. A significant association was found between oral candidiasis, denture stomatitis and the atrophic tongue with hyposalivation. Being woman was a risk factor for xerostomia and hyposalivation. <![CDATA[Conservative treatment for an adolescent with amelogenesis imperfecta.]]> https://scielo.conicyt.cl/scielo.php?script=sci_arttext&pid=S0719-01072019000300127&lng=pt&nrm=iso&tlng=pt RESUMEN: La amelogénesis imperfecta es un trastorno hereditario que afecta la formación del esmalte presentándose en dentición decidua y permanente. Existen numerosas clasificaciones, donde la amelogénesis imperfecta hipoplásica presenta las siguientes características clínicas: reducción del espesor del esmalte, coloración entre amarillo y marrón, superficie rugosa y falta de contacto interproximal. Estos pacientes reportan niveles más altos de evitación social y angustia por lo que es imperativo realizar un buen tratamiento rehabilitador. El presente artículo tiene como objetivo describir la secuencia terapéutica de un paciente adolescente diagnosticado con amelogénesis imperfecta hipoplásica utilizando la técnica modificada clear matrix con resinas compuestas.<hr/>ABSTRACT: Amelogenesis imperfecta is an inherited disorder affecting enamel formation in deciduous and permanent dentition. There are a large number of classifications, where hypoplastic amelogenesis imperfecta has the following clinical features: reduced enamel thickness, yellowish brown coloration, rough surface and lack of interproximal contact. These patients report the highest levels of social avoidance and distress, and it is therefore imperative to perform a good rehabilitative treatment. The objective of this paper is to describe the therapeutic sequence of an adolescent patient diagnosed with hypoplastic amelogenesis imperfecta, using the modified clear matrix technique with composite resins. <![CDATA[Gerontology in the pre doctoral academic training at the 21<sup>th</sup> century.]]> https://scielo.conicyt.cl/scielo.php?script=sci_arttext&pid=S0719-01072019000300131&lng=pt&nrm=iso&tlng=pt RESUMEN: La gerontología es la ciencia que trata de la vejez y los fenómenos que la caracterizan. Se aboca al estudio científico del proceso de envejecimiento y abarca la esfera social, psicológica y biológica. El fenómeno actual del envejecimiento de la población mundial, denominado “transición demográfica”, ha puesto al quehacer de la gerontología en acuciante actualidad. Dentro de su marco conceptual, en el ámbito de la ciencia médica, surge la geriatría, especialidad que se ocupa no sólo de la prevención y asistencia de las enfermedades que presentan las personas mayores, sino también de su recuperación funcional y de su reinserción en la comunidad. En Chile, sin constituirse en una especialidad propiamente tal, la llamada “odontogeriatría” vendría a ser el equivalente de geriatría en odontología. Sin embargo, creemos que gerodontología sería más pertinente puesto que apreciamos en ésta, una visión cultural derivada de la gerontología que va allende los aspectos procedimentales propios de la odontología. Pretendemos fundamentar esta afirmación, analizando la experiencia internacional, contrastándola con la situación que se verifica en Chile, reiterando la imperiosa necesidad de comenzar desde la formación de pre grado del futuro odontólogo con la entrega de insumos gerodontológicos teóricos y clínicos, para intentar aportar de modo propositivo y no efectista, en el diseño de políticas públicas actualizadas.<hr/>SUMMARY: Gerontology is the science studying ageing and the phenomena that characterize it. It deals with the scientific approach of this process encompassing the social, psychological and biological fields. The current phenomenon of the ageing of the world population, so called “demographic transition” has put the gerontological issue in pressing today. Within its conceptual frame and in the sphere of medical sciences emerges Geriatrics, specialty covering not only prevention and care of older people diseases, but also addressing its functional recovery and reintegration into the community. In Chile, not yet a specialty, geriatric dentistry would be its equivalent regarding odontology. However, we believe that there is a conceptual difference regarding gerodontology since we appreciate its scope goes far beyond the procedural aspects of dentistry. We will make our statement, analyzing evidence from international educational experiences, as well as contrasting with the situation that occurs in Chile, altogether stressing the urgent need to start from the undergraduate curriculum of future dentists through the delivery of theoretical input and clinical gerodontologic training, in order to collaborate in a proactive way in the design of updated public policies according to the nowadays challenges. <![CDATA[Contribution of dental adhesives in the rehabilitation of the edentulous patient. Narrative review.]]> https://scielo.conicyt.cl/scielo.php?script=sci_arttext&pid=S0719-01072019000300135&lng=pt&nrm=iso&tlng=pt RESUMEN: El uso de medios auxiliares como es el caso de los adhesivos dentales permitirá que la adhesión entre la prótesis y la mucosa sea mejor y se logre una mejor retención, estabilidad y eficiencia masticatoria. Se realizó una búsqueda en internet en las siguientes bases de datos, Pubmed, EBSCO, BEIC, Cochrane y Epistemonikos. Para la búsqueda electrónica se utilizaron las palabras “denture adhesive”, “dental adhesive”, “complete denture wearers”, “retention”, “support”, “stability”, “masticatory performance”, “Candida albicans” y “quality of life” relacionadas con el operador booleano AND. Al aplicar los criterios de inclusión y exclusión terminando con 14 artículos. Finalmente, quedaron 5 ensayos clínicos aleatorizados, 3 ensayos clínicos no aleatorizados, 2 estudios observacionales de cohorte prospectivos, 3 artículos in vivo y 1 estudio in vitro. Los adhesivos dentales han demostrado un mejoramiento en la retención, adhesión, estabilidad y eficiencia masticatoria en los portadores de prótesis totales, independiente de su presentación, sea en forma de polvo, crema, tiras o almohadillas. Por medio de su uso, se ha demostrado que los portadores de prótesis totales han podido mejorar su calidad de vida, tanto en la seguridad del manejo de su aparato protésico como en el aspecto nutricional, pues permite que se alimenten de mejor forma, esto incluso es posible observar cuando se prescriben en aquellos pacientes con prótesis recién instaladas.<hr/>ABSTRACT: Introduction: When performing the oral rehabilitation of edentulous patients using total prostheses, in some cases, a slight maladjustment of the prosthetic device within the oral cavity can compromise the chewing, swallowing and phonation functions. With the use of ancillary means such as dental adhesives, which allow the adhesion between the prosthesis and the mucosa, a better retention, stability and masticatory efficiency is achieved, and therefore, an increase in the quality of life of the patient. Main objective: To perform a narrative review to describe the contribution of the use of dental adhesives in patients with total dentures. Method: We searched the following databases, Pubmed, EBSCO, BEIC, Cochrane and Epistemonikos. The words “denture adhesive”, “dental adhesive”, “complete denture wearers”, “retention”, “support”, “stability”, “masticatory performance”, “Candida albicans” and “quality of life “ were searched electronically with the Boolean operator AND. Results: As a result, 394 articles were obtained, 85 of which were selected by title, of which the repeated articles were eliminated, leaving 32; then, the abstracts were read, leaving 22 articles. The remaining articles were read completely, the inclusion and exclusion criteria were applied, ending with 14 articles. Finally, 5 randomized clinical trials, 3 non-randomized clinical trials, 2 prospective cohort observational studies, 3 in vivo articles and 1 in vitro study were included. Conclusion: Dental adhesives have showed an increase in retention, stability, ability and masticatory efficiency in patients with total prostheses, regardless of their presentation. Through its use, it has been demonstrated that total denture wearers have been able to improve their quality of life, even patients with recently installed prostheses. <![CDATA[Elian class 3 regeneration of soft tissue using a laterally moved flap and connective tissue graft. A clinical case report]]> https://scielo.conicyt.cl/scielo.php?script=sci_arttext&pid=S0719-01072019000300140&lng=pt&nrm=iso&tlng=pt RESUMEN: Se describe el caso clínico de una paciente de 60 años, sexo femenino, sana, la cual presentaba en el diente 1.1 una recesión de 6 mm de longitud con extensa pérdida ósea en vestibular correspondiente a una clase 3 de Elian. En el presente reporte se expone los pasos que se siguieron para llegar a una Clase 2 de Elian, mediante un colgajo desplazado lateral con injerto de tejido conjuntivo subepitelial.<hr/>ABSTRACT: We describe the clinical case of a 60-year-old female patient, healthy, who presented a 6mm-long recession on tooth 1.1 with extensive vestibular bone loss corresponding to an Elian class 3. In the present report, the steps followed to reach an Elian class 2,by means of a laterally moved flap with subepithelial connective tissue graft, are shown. <![CDATA[Central Odontogenic Fibroma of the Mandible: A Short Literature Review Apropos of a Case]]> https://scielo.conicyt.cl/scielo.php?script=sci_arttext&pid=S0719-01072019000300144&lng=pt&nrm=iso&tlng=pt RESUMEN: El fibroma odontogénico central (FOC) es una neoplasia benigna poco frecuente que representa alrededor del 1.5% de los tumores odontogénicos intraóseos. Su presentación es exclusiva de huesos maxilares. Tiene origen mesodérmico, pudiendo derivar del folículo dentario, ligamento periodontal y/o la papila dental. Tradicionalmente presenta dos variantes histológicas: un tipo que es pobre en epitelio y otro tipo que es rico en epitelio con focos de material calcificado. En la mayoría de los casos muestra un crecimiento lento y progresivo que produce expansión cortical con o sin sintomatología. Radiográficamente es común observar una imagen radiolúcida y unilocular, raramente se pueden exhibir lesiones multiloculares y/o de radiolucidez mixta. El tratamiento indicado en la mayoría de los casos es la enucleación del tumor. El siguiente reporte de caso describe las características clínicas, imageneológicas e histopatológicas de una lesión mandibular con diagnóstico de FOC y cuyo tratamiento realizado fue la enucleación del tumor.<hr/>ABSTRACT: Central odontogenic fibroma (COF) is a rare benign neoplasm that accounts for about 1.5% of intraosseous odontogenic tumors. Its presentation is exclusively in the maxillary bones. It has a mesodermal origin, being able to derive from the dental follicle, periodontal ligament and/or the dental papilla. Traditionally, it has two histological variants: one type that is poor in epithelium and another type that is rich in epithelium with foci of calcified material. In most cases it shows a slow and progressive growth that produces cortical expansion with or without symptomatology. Radiographically, it is common to observe a radiolucent and unilocular image, but rarely multilocular lesions and / or mixed radiolucency can be exhibited. The treatment indicated in most cases is the enucleation of the tumor. The following case report describes the clinical, imaging and histopathological characteristics of a mandibular lesion diagnosed with FOC and whose treatment was enucleation of the tumor. <![CDATA[Burkitt lymphoma associated with Human Immunodeficiency Virus. A clinical case report.]]> https://scielo.conicyt.cl/scielo.php?script=sci_arttext&pid=S0719-01072019000300148&lng=pt&nrm=iso&tlng=pt RESUMEN: El linfoma de Burkitt es un linfoma de células B maduras altamente agresivo, de baja incidencia. Según la Organización Mundial de la Salud presenta tres variantes: endémico, esporádico y asociado al Virus de la Inmunodeficiencia Humana. Este último representa el 30-40% de los linfomas no Hodgkin en pacientes seropositivos para virus de la inmunodeficiencia humana. El tratamiento con quimioterapia en combinación a terapia antirretroviral de gran actividad da muy buenos resultados. Se reporta caso de paciente seropositivo para virus de la inmunodeficiencia humana con linfoma de Burkitt en la cavidad oral, tratado con la terapia anteriormente mencionada.<hr/>ABSTRACT: Burkitt lymphoma is a highly aggressive mature B-cell lymphoma with a low incidence. According to the World Health Organization, it has three variants: endemic, sporadic and associated with Human Immunodeficiency Virus (HIV). The last one represents 30-40% of non-Hodgkin’s lymphomas in HIV seropositive patients. Treatment with chemotherapy in combination with highly active antiretroviral therapy gives very good results. Here is the case report of a HIV seropositive patient who developed Burkitt lymphoma in the oral cavity and received this treatment. <![CDATA[Remodeling interdental gingival papilla with hyaluronic acid. An aesthetic solution]]> https://scielo.conicyt.cl/scielo.php?script=sci_arttext&pid=S0719-01072019000300151&lng=pt&nrm=iso&tlng=pt RESUMEN: El objetivo de este trabajo es evaluar el efecto de remodelación y relleno mediante aplicación de ácido hialurónico en papilas gingivales de sector estético con defectos de triángulo negro en un paciente tratado periodontalmente. Se inyectaron 0.15 ml de gel de ácido hialurónico dividido en 3 sesiones. Clínicamente se observó aumento de volumen y aumento de dimensiones verticales y horizontales en las papilas, mejorando así los parámetros estéticos. Como conclusión la aplicación de ácido hialurónico es efectiva en la remodelación y relleno en pérdidas de papila clase I de Nordland y Tarnow.<hr/>ABSTRACT: The objective of this work is to evaluate the effect of remodeling and filling through the application of hyaluronic acid in gingival papilla in the aesthetic sector with black triangle defects, in a periodontally- treated patient. In 3 portioned sessions, 0.15 ml of hyaluronic acid gel was injected. Clinically, an increase in volume was observed, and vertical and horizontal dimensions in the papilla increased too, thus improving aesthetic parameters. In conclusion, the application of hyaluronic acid is effective in the remodeling and filling in losses of class I papilla (Nordland and Tarnow classification). <![CDATA[Socket-shield technique for the preservation of alveolar ridge. Clinical case report.]]> https://scielo.conicyt.cl/scielo.php?script=sci_arttext&pid=S0719-01072019000300154&lng=pt&nrm=iso&tlng=pt RESUMEN: Algunas técnicas como la regeneración ósea guiada, uso de rellenos óseos y membranas se han planteado para limitar la reabsorción ósea post exodoncia. La técnica de “socket-shield” preserva un fragmento de raíz vestibular como medio para preservar la cortical ósea bucal. El presente reporte describe un caso clínico de fractura coronaria en un incisivo central superior que fue segmentado parcialmente y la porción vestibular de la raíz fue preservada para mantener la arquitectura de la cortical vestibular, al mismo tiempo se colocó un implante dental. Posterior a la rehabilitación fija, y con un seguimiento de un año se evidenció la preservación de los tejidos blandos periimplantarios a nivel de la mucosa y tejido óseo. No se observó alteraciones en la oseointegración y la pérdida ósea marginal periimplantaria fue nula. Se concluye que la técnica de “socket-shield” parece conservar clínicamente la arquitectura de la mucosa y el tejido óseo sin ocasionar patologías periimplantarias.<hr/>ABSTRACT: Some techniques such as guided bone regeneration with the use of bone fillings and membranes have been proposed to limit bone resorption. The socket-shield technique preserves a fragment of the vestibular root as a means of preserving the buccal bone cortex. This report describes a clinical case of coronary fracture in an upper central incisor that was partially segmented and the vestibular portion of the root was preserved to maintain the architecture of the vestibular cortex; at the same time, a dental implant with immediate provisionalization was placed. After the fixed rehabilitation and with a follow-up of one year, the preservation of the soft peri-implant tissues at the level of the mucosa and bone tissue was evidenced. No alterations in osseointegration were observed and marginal peri-implant bone loss was null. It is concluded that the socket-shield technique seems to clinically conserve the architecture of the mucosa and bone tissue without causing peri-implant diseases. <![CDATA[Hyaluronic acid for repairing interdental papilla in esthetic area: case report.]]> https://scielo.conicyt.cl/scielo.php?script=sci_arttext&pid=S0719-01072019000300157&lng=pt&nrm=iso&tlng=pt ABSTRACT: Background: Hyaluronic acid is a treatment option for this deficiency gingival papilla, since it aims to increase tissue volume in the region of loss of interdental papilla, minimally invasive. Aim this study was to describe the application of Hyaluronic acid in a deficient gingival papilla. Case report: Patient presented absence of interdental papilla in the maxillary arch around upper anterior teeth. A comprehensive exam was performed, plus 2D photographs taken with a professional camera coupled to a device for image standardization, and an intraoral scanning with the OMNICAM scanner. Three applications of Hyaluronic acid were performed to fill the black spaces, at an interval of four weeks. The patient was followed-up for three months, and the recovery of papilla was observed. Conclusion: The Hyaluronic acid as gingival filling material can be considered as a treatment option for augmenting interdental papilla in aesthetic area.
