The midcervical fissure is a rare congenital anomaly which affects the skin and subcutaneous tissue in the anterior region of the neck. Various pathological mechanisms have been proposed to explain this lesion, essentially produced by a failure of fusion of the lateral prominences of the 2nd brachial arch in the cervical midline, occuring between the 5th and 6th week of gestation. Only about 100 cases have been reported in the published literature and correspond to approximately 1.7% of all congenital cervical malformations. We describe 3 cases of this lesion, detailling the clinical presentation, its surgical correction by completely excising the compromised tissue, primary closure and its posterior evolution
