We report the case of a 34 years old healthy man who had had a viral hepatitis of unknown etiology ten years before. He refered a history of a week of malaise, headache, fever and a few days later he developed pain in the right hypocondrium, jaundice and choluria. After admission, he presented with a maculopapular exanthem which involved palms and soles. Laboratory tests showed leukopenia, low platelets and high levels of bilirrubin and transaminases. The VDRL test was positive ¼ and MHA-TP test was positive also. The investigation for hepatitis virus A, B, C, citomegalovirus and Epstein Barr virus was negative. Only IgG for hepatitis E virus was positive. Finally the liver biopsy evidenced alterations concordant to syphilitic hepatitis. The patient was treated with benzathine penicillin 2.400.000 U.I once a week, for three weeks with a good response to treatment. Then a positive polymerase chain reaction for human immunodeficiency virus (HIV) was received. This case shows an unusual manifestation of secondary syphilis in a HIV positive patient whose HIV status was unknown at the moment of diagnosis
