We report a 50 years old patient, with history of obesity and hypertension, who presented at our clinic with recurrent flank pain. A 2.5 cm stone was documented in a calyceal diverticulum in the lower calyx of the right kidney. On an abdomen CAT scan, an incidental 3 cm. mass was found in right the adrenal gland. The work-up studies confirmed the diagnosis of primary hyperaldosteronism. The patient was placed in left flank position, pneumoperitoneum was created and a 4-port laparoscopic adrenalectomy was performed. The patient was then placed in a modified lithotomy position and a right ureteral open-end catheter was placed cystoscopycally. Under fluoroscopic guidance, the right lower pole renal calyceal diverticulum was punctured with an 18-gauge needle. Conventional tract formation was performed and percutaneous nephrolithotripsy (PCNL) methods were used to remove the stone endoscopically. Laparoscopic adrenalectomy and PCNL operative time were 60 minutes each. The hospital stay was 48 hours. At 30 months follow-up the patient is asymptomatic, stone-free and with normal blood pressure levels
